If you have experienced or witnessed discrimination, harassment, or sexual misconduct by an employee, student, or visitor at IU Indianapolis, IU Columbus, IU Fort Wayne, or IU School of Medicine, please contact the IU Indianapolis Office of Institutional Equity (OIE).

Make A Report

Report Faculty / Staff / Student / Visitor Conduct to OIE

Report a discrimination, harassment, or sexual misconduct incident(s) to the Office of Institutional Equity involving faculty, staff, students, or visitors.

Report Student Conduct

Report academic or personal misconduct by a student to the Office of Student Conduct in the Division of Student Affairs.

Report Activities to EthicsPoint

Report activities related to financial, research, fraud, information technology, or other policy violations through IU's EthicsPoint.

Report Bias

If you have experienced bias or hate, please complete the IU bias incident reporting form.

File a Human Resources Grievance

For matters related to human resources, please review the grievance procedures.

Reporting Concerning Behavior

If you want to report concerning behavior of a student, faculty, or staff member, fill out a Behavioral Consultation Team (BCT) form to connect with the team.
